Patella fracture - initial therapy program (first 6 weeks)

Patella or the knee cap is a small bone situated at the front of the knee joint with the purpose to protect it because without it the knee would be fairly open to blows and unprotected. Together with the quadriceps tendon, the patellar tendon and the quadriceps muscle it forms the knee joint extensory mechanism. During knee extension the patell is pulled, ie. it slides up and down on the knee joint surface.

Due to its position and the fact thta it is exposed during falls and direct blows to the knee, patella fractures can be very common. They happen during falls, eg. falls from the bicycle or motorcycle, falls after slipping on ice, but they are also common after car accidents. Statisticaly they are most likely to occur in the 20 to 50 years age span and are more common with the male population, probably due to the fact that men more often participate in activities where falls and consequantial patella fractures can occur.

Treatment can be through conservative or surgical approach, depending on the type and gravity of the fracture. Patella fractures can be partial or complete, dislocated or stable, located at the proximal, medial or distal third of the patella, vertical or transverzal, single or multipl (eg. comminuted fracture, the bone shatters), open or closed. With more difficult fractures, open, multiple and dislocated fractures, a surgical approach is implemented.

Conservative treatment uses immobilisation of the injured leg, lasting from 4 to 6 weeks in order to let the bone heal and later in the functional phase when the immobilisation is removed and the bone has properly healed, knee range of motion, upper leg muscle strenght and knee stability should be regained progressively. After a surgical approach the leg is alos put into either a cast or an orthosis which limits knee joint movement by keeping the leg locked in a straight position. The limitations of the movement in the knee can be reduced during time by the doctor who performed the surgery. The functional phase  of rehabilitation after a surgical approach has a slightly slower progresion and last longe, but the principles remain the same as with the conservative approach. Knee range of motion, upper leg strenght and knee stability should be fully regained.

INITIAL PATELLA FRACTURE PROGRAM

{     - during first 6 weeks post surgery or injury in case an orthosis, which can be removed, is used to ensure immobilisation of the knee so the patella could heal properly

      - use of physical therapy methods to reduce pain, swelling and possible hematoma
